Important information for parents and guardians:

Required Immunizations:

Indiana State law requires all students to have a current immunization statement on file with the school.  See below for the 2023-24 required immunizations.

Parents should call their family's physician or the Putnam County Health Department at (765) 658-2782 to make appointments before school starts.

Just a reminder, the Religious Objections to Vaccinations form must be completed annually. If you need a new form, please follow the link below or pick up a form from one of the school offices. Please return the completed form to your student’s school.

Medications:  

We strongly recommend medications be given at home when at all possible. Students are not permitted to carry prescription or over-the-counter (OTC) medication while on the school bus or at school. Exceptions will be made if parents provide the school with a signed statement from physician indicating the student may carry an inhaler, EpiPen, or insulin at school. It is strongly recommended that parents or guardians bring in and pick up all prescription and OTC medications to be given at school.

Prescription medications must be in the original bottle with the name of the child, as well as, the type and dosage of the medication clearly visible.  The medication label must match the physician's order, and the time of day medications are to be administered must also be on the label. Medications not picked up at the end of the school year will be disposed of.

With the advent of online registration, it is important that the parent or guardian fill out online registration forms and not the student where medications are concerned.

Health Conditions:

If your child has any diagnosed health conditions such as asthma, diabetes, seizures, or allergies requiring an EpiPen, it is very important to call or come in and meet with your school nurse at the beginning of the school year, or anytime your child has a change in condition.

Mandatory Hearing and Vision Testing:

Hearing tests will be given yearly to students in grades 7 and 10 and/or by the request of a student or teacher. Vision tests will be given yearly to students in grades K, 3, 5, and 8 and/or by the request of a student or teacher. Parents who do not want their child tested, please send in a written note stating that request.
