Please see the updated list for 2020-2021 student vaccination requirements by clicking on the document below.

Also...if your child takes prescribed medications during the school day, you will need to complete the two additional forms and return them to Tiffany Shields, CCSC Corporation Nurse, or Cristle Grove, CHS LPN.
